Its on that page u sent on the left-hand under Notice and Claim form. U have to open it with Adobe,the site provides it if u dont have it. wow u can send specified serial numbered ps2s and trade it for a new one for $41US if you fit in to these benifets. ur going to have to read it good.
Too bad I opened mine to fix it(Damn XPLAY).:mad:

lol yeah me too....have to be one of those guys who follows the warranty and all that, bah, warranty smoranty...i already but another one =(
